Key Information

The planes are like a stack of plates: they reside one on top of the other on top of the other
When one is on a plane, you cannot see the planes above or below you; they're like invisible
Exception: when there is a portal, you can see through to the next plane
Exception: There are a few areas where one plane's caves reach deep enough underground to reach the land of the plane below it. If one goes to that intersection, you obviously can see the plane next to the one you are on.
Because the other planes are invisible to you, you can see the astral bodies straight through the land, people, oceans, etc.
Surrounding this stack of "plates" is the void in which the major deities were formed
Demiplanes are kind of like epicycles (see this diagram). The big circle is the main stack, but the epicycles are the demiplanes.
When a new, movable demiplane is created (for example, the creation of a Bag of Holding), the "smaller plate" follows around the holder around.
A demiplane's placement in the stack is dependent upon the intention of the creator(s)
If the will of the creator is neutral or good/positive, the "plate" will be above the plane the caster/object is on
If the will of the creator is evil/negative/bad, the "plate" will be below the plane the caster/object is on

The Major Planes

The list is ordered from highest plane to lowest plane. All of the planes are flat, with the exception of the Ethereal Plane.

1. Ethereal Plane

Imagine pretty images of space. That's basically what this looks like. This plane has the clearest view of the stars, as there is no weather, nor is there any plane blocking your vision. This is the only plane that has no ground, one just floats around.

While you can still float downwards, you cannot float "down" and reach the Elemental Planes. You have to use some sort of magic to travel to the lower planes

This is the only plain that is not flat; The full shape of the Ethereal Plane is unknown, but what is known is that it acts as a bubble surrounding the lower planes. This is what stops people from "falling off the edge" of the lower planes.

2. Elemental Plane

The Elemental Plane is split roughly up into quarters: the Air, Earth, Fire, and Water quarters. In each quarter, the corresponding elementals are the main inhabitants.

3. Feywild

The Feywild is one of two "echoes" of the Material Plane.

All "Fey" creatures (Eladrin, Saytrs, etc) are native to this plane.

4. Material Plane - Everania

This is the plane that most important events happen on (at least to the players)

5. Shadowfell

Also known as the "Underdark"

Shadowfell is the other "echo" of the Material Plane

6. The Hells

This plane is the combination of the official D&D lore's Abyss and Nine Hells

The two major factions in The Hells are Demons (Abyss) and Devils (Nine Hells)
